Apply for the Apollo/Alumni Achievement Award
Please read ALL of the instructions and follow them carefully. All components must be received by the March 2nd deadline.
The application is a fillable pdf that must be completed online and printed. Handwritten applications will not be accepted.
Question #3 asks for the GPA. For us to verify this, you must order an official copy of your transcripts and have the sealed transcript mailed to:

You will need to answer questions 4-9 succinctly.
When you have completed the form, select "PRINT". Mail or deliver the application, along with the required letters of recommendation, to the Student Activites Coordinator for your campus.
