All claim reports begin with a source document identified as the Accident/Incident Report. An Accident/Incident report must be completed for all incidents on SPC property. Fax completed report forms and any supporting documentation to Risk Management for review and processing. The Accident/Incident report form can be found by going to Staff Central.
FACILITIES USE/SPECIAL EVENTS LIABILITY COVERAGE
This coverage provides a mechanism to protect the colleges’ self-insured loss fund when the college allows its facilities to be used (rented) by non-college organizations for non-college sponsored activities. Coverage through this program is fully insured and the cost is reimbursed by the organization renting the facility. Coverage under this plan is not automatic-all events are subject to approval by the carrier. A site representative has been designated at most SPC sites to coordinate rentals. Renters will provide proof of insurance for their event or apply under this program, unless exempted from this requirement by the College President. If the renter requests coverage under this policy, the site representative will send the Facilities Use/Special Event Liability Coverage form to Risk Management to process the request. Risk Management is responsible for tracking usage of this policy college-wide.
Athletic injuries must be submitted to the insurance carrier within 30 days of the incident using the Accident/Incident report form. The form needs to be signed by the coach and the Athletic Director prior to being sent to Risk Management for submission to the Insurance Carrier. This coverage is designed to be excess coverage and pays after primary insurance coverage has paid it's portion. The student must also complete a claim form.
Allied Health injuries must be submitted to the insurance carrier within 30 days of the incident using the Accident/Incident report form. The form needs to be signed by the on-site supervisor, and the program director prior to being sent to Risk Management for submission to the Insurance carrier.
STUDENT ACCIDENTS
Injuries to students participating in an Allied Health clinical program must be submitted to the insurance carrier within 30 days of the incident using the Accident/Incident report form. The student must also complete a claim form.
VEHICLE ACCIDENTS WHILE ON COLLEGE BUSINESS
College owned vehicles are covered for auto liability through the FCCRMC Consortium.
When staff and faculty drive personal vehicles while on college business, it is important to check with your personal insurance carrier regarding your vehicle
coverage!
Personal Injury Coverage-NO coverage is provided by the College or its insurance carrier for the owner of the vehicle, as they must look to their personal automobile policy for coverage. Under workers’ compensation – employee injuries occurring while on the job would be covered in accordance with workers’ compensation law.
Property Coverage - College owned property (tools, etc.) is covered up to $2.5 million subject to deductible of $10,000 per occurrence ($5,000 deductible per item for theft subject to $10,000 per occurrence deductible) when property is kept in staff/faculty personal vehicles. Refer to the insurance plan document for those items not covered such as animals, accounts, bills, valuable papers, etc. There is NO coverage for personal property of employees—they would look to their Homeowners coverage.
Damage to employee’s vehicle is NOT covered—they must look to their personal automobile policy for coverage.
