Non-Emergency Treatment

 

After Business Hours:

Monday - Friday after 5 p.m.

Saturday and Sunday all day

 

1. Contact Security immediately at ext. 2560 from any College phone or from an outside line 791-2560 to report the accident/incident.

Fax the Accident/Incident report to Risk Management 341-3093 and give the original to your supervisor for signing.

2. Report to the nearest Emergency Room for Treatment.

If you can not reach anyone and need to have treatment, you must notify your supervisor and Risk Management as soon as possible.

3. You will be required to report to Lakeside Occupational Medical Center on the next business day.  You must contact Risk Management at 341-3080 or ext 3080 from any college phone for pre-authorization and notification that you went to an emergency facility.  

Monday morning is considered the next business day for Friday after 5:00 p.m., Saturday, and/or Sunday.

4. Please make the hospital aware that you are there for a Workers Compensation injury and give them the following contact information:
