The HMO requires that members choose their dentists from a specified list; however, each covered member may choose a dental facility. This benefit also includes co-pays for orthodontia benefits and specialists.

The PPO pays benefits regardless of the dentist.  This plan is subject to deductibles, co-insurance factors, and annual benefit maximums. Allowable charges are based on charges being made by providers in the area where the dental services are being performed. This is a limited benefit plan - pre-existing conditions may not be covered.
