FAFSA tools

Verification

  • Asset Worksheet
    Use this form to report asset information if required to do so on your To Do List.
  • Dependency Status Worksheet
    Complete if you have been asked by Financial Assistance Services for documentation regarding your dependency status for 2012-2013.
  • Dependent Clarification Worksheet
    Use this form if you (the student) or your parent(s) included dependent(s) in the household size that were not claimed as a dependent on your or your parents’ 2011 tax return.
  • Grade Level Correction
    After you have corrected your grade level information on your FAFSA, submit this form if you have been instructed to do so. FAS will use it to assess your eligibility for financial assistance as a dependent student.
  • Income Clarification Worksheet
    Used during the verification process for students who have no reported income or are not required to file taxes.
  • Pell Grant Grade Level Correction
    After you have corrected your grade level information on your FAFSA, submit this form if you have been instructed to do so. FAS will use it to assess your eligibility for a Pell Grant.
  • Selective Service Verification Form
    For students with the checklist item asking for proof of registration with the Selective Service, this sheet will help you understand what information and documents our office needs.
  • Student Aid Eligibility Worksheet for Question 23
    Use this worksheet to determine your eligibility for financial aid if you have been convicted of possessing or selling illegal drugs or because you left question 23 on the FAFSA blank.
  • Verification Worksheet
    If you are selected for Verification, please submit this form, along with 2011 tax information for the student and/or parents.
  • Request for Transcript of Tax Return
    Use IRS form 4506-T to request that a paper copy of your return be sent to your personal address. Please do not submit this request to SPC and do not have the Tax Return Transcript mailed directly to SPC.

Fee Waivers

  • Department of Children and Families Tuition and Fee Exemption
    Students who received services from the Department of Children and Families (DCF) may be eligible for a Tuition and Fee Exemption if they reached age 18 in the custody of DCF, were adopted from DCF, were in the custody of a relative under s. 39.5085, F.S. or placed in a guardianship by the court after reaching age 16 and spending at least 6 months in the custody of DCF.
  • Fee Waiver for Purple Heart
    Students who have received a Purple Heart or other combat decoration superior in precedence may be eligible for a fee waiver.
  • Homeless Fee Waiver
    If you are a Homeless Fee Waiver recipient, complete this form if you have registered for classes. Complete this form again if you change or alter your class schedule for the term.
